Take it upon yourself to get information from the doctor

Ask for the truth, if necessary demand it, if you really want it. If you need to get a 2nd opinion, go to another doctor. As a consumer of health care the service you receive is partly your responsibility. You can not leave yourself at the mercy of what the doctor thinks would be best for you if you happen to think otherwise - in terms of knowing about a terminal illness for example.
