The Let’s Get Healthy California framework identifies six goals with specific indicators to advance the Triple Aim and to promote greater health equity.

We are looking for submissions that:
- Promote health equity
- Showcase use of data
- Engage community partners
- Help drive measurable health outcomes
- Are rooted in strong research or theoretical evidence
- Demonstrate credibility and influence in their targeted communities
- Provide scalable approaches to be used in different community settings
