Trinity Health provides care to more than 207,000 dual eligible patients, representing $1.5 Billion in acute care revenue annually. Similar to national trends, Trinity Health has a high 30-day and 90-day readmission rate for dual eligible patients. In Trinity Health Medicare Shared Savings Program (MSSP) Accountable Care Organizations (ACOs) readmission rates for all dual eligible patients is 138 per 1,000 compared to 67 per 1,000 for non-duals. Trinity Health's 90-day readmission rates related to Bundled Payment for Care Improvement (BPCI)-triggering episodes are 37% higher for duals than non-duals. With the changes in clinical practices and reimbursement, this population will likely benefit from new models of care and technologies that improve post-discharge and outpatient services and prevent readmissions.

We are seeking solutions that will enable Trinity Health to decrease our high 30-day readmission rate for dual eligible patients in ACOS and our high 90-day readmission rate for dual eligible patients who participate in bundles.
