Screening instrument (PraPlus)

Pra and PraPlus are screening tools that identify elders who are at high risk for using healthcare services heavily in the future. Pra is 8-item questionnaire and PraPlus has additional 9 items that evaluate older persons� medical conditions, functional ability, living circumstances, nutrition and depression.
