Child Health & Illness Profile - Child Edition (CHIP-CE), Child Report Form & Parent Report Form

The Child Health and Illness Profile (CHIP) is a family of self-report questionnaires designed to assess the health of children (Child Edition-CE) and adolescents (Adolescent Edition-AE) ages 6 through 17 years. The CHIP-CE is a generic child health status questionnaire that comprehensively describes all aspects of child health that can be influenced by the health care and school systems - satisfaction, discomfort, resilience, risk avoidance, achievement, and disorders. The Child Report Form (CHIP-CE/CRF) is fully illustrated with cartoon-type characters that anchor the 5 response options for each item. The children answer 45 health items and 4 demographic items. Two parent versions are available: The Standard Parent Report Form (PRF) has the same 45 items as the child version and 7 demographic items. The Comprehensive PRF has an additional 31 health items (total 76) to obtain more detail and allow subdomains to be scored. An additional Disorders module is available that allows parents to identify the child�s acute and chronic conditions (41 items). The domains and subdomains were conceptually derived and generally supported by factor analysis.
