Sickness Impact Profile" a/k/a "SIP (Copyright Only)

Sickness Impact Profile (SIP) is an assessment tool that consists of 136 questions and measures behaviorally-based health status. It has been validated and used for over 30 years. The SIP can be self- or interviewer-administered and asks respondents to endorse items that describe their everyday activity and are related to their health. Scoring is based on the number and type of items endorsed.
