Johns Hopkins Diabetes Management Program
The invention encompasses a team of health care professionals who directly manage the pharacologic, nutritional and educational components of the medical care plan for hospitalized patients with diabetes mellitus. The director of the team wil be a board certified endocrinologist, who wil be responsible for oversight, algorithm development and interfacing with other disciplines (administrative, nursing, pharacy, case managers etc). A nurse practitioner(s) wil provide the day-to-day management of glucose control for the hospitalized patient. A diabetes educator(s) wil provide counseling regarding nutrition, blood glucose monitoring, insulin injections and complications of diabetes. A data analyst wil provide expertise in establishing, maintaining and generating data for quality assurance and financial outcome measurements.Type of Offer: Licensing
