Hospital at Home Care Model

Acute hospital care can be burdensome for elderly patients. Providing acute hospital-level care to a patient in his home in lieu of an acute hospital admission may improve patient outcomes and be less expensive. These protocols provide organizations with sufficient information to understand, plan, and implement, a Home Hospital Program that provides a home-based alternative to acute hospitalization for older persons.
